Description

When a pressure relief valve on a sodium hydroxide feed system opened, it released a small stream of caustic soda. The valve failed to reseat completely, causing the stream to continue flowing for more than 48 hours. The stream of caustic soda was discharged into Coyote Creek. Some cyclists riding through the creek were injured. This set off a series of events that would involve studies at more than 30 facilities; eventually, $40 million was appropriated to minimize the possibility of such a spill recurring. This paper describes the utility’s effort to that end.
